Membership Form 2008

To join Orca Swim Team for 2008, we invite you to fill out the form below. You now have the option of paying on-line using PayPal, in addition to paying by check.

Alternatively, you can print out a PDF of this same form by clicking here. You can, then, mail it in with your $15 until December 31, 2007. Thereafter, the membership donation goes up to $20.

(USMS registration is required to swim with the Orcas at Connelly Center. You can register for USMS using the form available from the SwimPNA website: www.swimpna.org.)

A team roster is mailed out with newsletters to make it convenient to contact each other. If you leave this section blank, your name, email, and phone number will be listed on the roster.
